Schedule A: Health, Safety and Informed Consent

Supplement to Terms and Conditions

Effective Date: June 2026

Introduction

Jillian Blair Skin offers a range of cosmetic skincare treatments, some of which involve advanced devices, chemical formulations, or physical manipulation of the skin. The purpose of this Schedule is to disclose the specific risks associated with each category of service so that you can make an informed decision before receiving treatment.

Our services are cosmetic in nature and are not medical treatment, diagnosis, or therapy. No outcome is guaranteed. Results vary based on individual skin type, health history, lifestyle, medication use, aftercare compliance, and other factors outside our control.

If you have a medical condition, take prescription medication, are pregnant or nursing, or have recently undergone a medical or cosmetic procedure, you must disclose this before your appointment. We may require medical clearance before proceeding with certain treatments.

1. General Risks Applicable to All Services

All cosmetic skincare treatments carry a baseline level of risk. Known and foreseeable risks that may occur with any service include, without limitation:

  • Temporary redness, warmth, or flushing of the skin

  • Sensitivity or discomfort during or after treatment

  • Dryness, tightness, or mild peeling

  • Temporary breakouts or purging, particularly during early phases of a new protocol

  • Allergic or adverse reactions to products, ingredients, or materials used

  • Temporary worsening of the skin concern being addressed before improvement occurs

  • Pigmentation changes, including temporary darkening or lightening of treated areas

  • Swelling or bruising in areas involving extraction or physical manipulation

Risk level varies based on your skin type, fitzpatrick tone, current health status, medications, recent sun exposure, recent procedures, and aftercare compliance. You are responsible for accurately disclosing all relevant information before treatment.

2. Facial and Cleansing Treatments

Services: JBS Signature Facial, Clarifying Facial, Pre-Natal Facial

Description: These treatments involve cleansing, exfoliation, masking, massage, and product application. The Clarifying Facial includes high-frequency device use and LED light therapy. The Pre-Natal Facial uses pregnancy-safe formulations only.

Service-Specific Risks

  • Extractions: temporary redness, swelling, bruising, or post-extraction breakouts in treated areas

  • High-frequency device: contraindicated for clients with pacemakers, epilepsy, pregnancy, or metal implants in the face

  • LED light therapy: contraindicated for clients with photosensitivity conditions or those taking photosensitizing medications

  • Enzyme exfoliation: may cause sensitivity or reaction in clients with compromised barrier function or known ingredient sensitivities

Pre-Natal Specific

The Pre-Natal Facial is formulated to avoid actives contraindicated in pregnancy. However, you are responsible for disclosing your pregnancy status, trimester, and any pregnancy-related complications. JBS does not provide medical advice, and you should consult your obstetrician before booking any skincare treatment during pregnancy.

3. Microcurrent Treatment

Service: Ageless Microcurrent (MyoLift 600)

Description: Microcurrent delivers low-level electrical current to facial muscles to stimulate, lift, and tone. It is a non-invasive, device-based modality.

Service-Specific Risks

  • Mild tingling or muscle twitching during treatment

  • Temporary skin sensitivity or redness post-treatment

  • Contraindicated for clients with: pacemakers or implanted electrical devices, epilepsy, active cancer (particularly in the treatment area), pregnancy, active infection or open lesions in the treatment area, or metal implants in the face or neck

  • Not recommended for clients who have received Botox or dermal filler within two weeks of treatment

You must disclose any of the above contraindications before your appointment. Treatment may be declined or modified based on this information.

4. Hydrodermabrasion

Service: Diamond HydroFacial (Bio-Therapeutic Bio-Brasion), Diamond HydroFacial + Dermaplaning, Diamond HydroFacial + Microcurrent

Description: Diamond-tip hydrodermabrasion mechanically exfoliates the skin using suction and a diamond abrasion tip. Combination services layer additional modalities including dermaplaning and microcurrent.

Service-Specific Risks

  • Redness, temporary sensitivity, or skin tightness following treatment

  • Increased sun sensitivity for 24-72 hours post-treatment. SPF use is mandatory.

  • Possible temporary surface breakouts as congestion is cleared

  • Bruising or capillary sensitivity in clients with rosacea, sensitive skin, or compromised barrier function

  • Contraindicated for: active acne lesions (open, inflamed, or cystic), active rosacea flare, recent chemical peel or laser treatment within two weeks, sunburn, use of isotretinoin (Accutane) within six months, blood-thinning medications (may increase bruising risk), open wounds or skin infections in the treatment area

5. Dermaplaning

Service: Dermaplane Glow, Diamond HydroFacial + Dermaplaning

Description: Dermaplaning uses a sterile surgical scalpel to manually exfoliate the outermost layer of dead skin cells and remove vellus hair from the face. The procedure is performed by a trained esthetician.

Service-Specific Risks

  • Redness, sensitivity, or tightness immediately following treatment

  • Nicks or irritation if the skin is particularly dry, thin, or reactive at the time of service

  • Increased sun sensitivity post-treatment. SPF use is mandatory for a minimum of 72 hours.

  • Contraindicated for: active acne lesions (open or inflamed), active cold sores, rosacea flare, inflammatory skin conditions, use of isotretinoin within six months, blood-thinning medications, excessively dry or compromised barrier skin

Vellus hair grows back at the same rate and texture. Dermaplaning does not change the rate, color, or type of hair regrowth.

6. Nano-Needling / Micro-Channeling

Service: Vitamin-A Facial Infusion (Revita Pen Pro)

Description: This treatment uses nano-needling technology to create micro-channels in the skin's surface, allowing deeper penetration of Vitamin A, stem cell factors, Vitamin C, and custom-blended actives. It is distinct from traditional microneedling and does not penetrate to the dermis.

Service-Specific Risks

  • Redness and mild sensitivity for 24-48 hours post-treatment

  • Temporary skin tightness or flaking as actives absorb and skin renews

  • In rare cases, temporary breakout or purging as cell turnover increases

  • Increased sun sensitivity for a minimum of 72 hours. SPF use is mandatory.

  • Contraindicated for: active acne lesions (open or cystic), active cold sores or herpes simplex in the treatment area, recent chemical peel or laser treatment within two weeks, isotretinoin use within six months, pregnancy (Vitamin A is a known teratogen), open wounds, infection, or compromised skin integrity in the treatment area

This modality triggers an active collagen response over approximately 30 days. Results are progressive and not immediate. Multiple sessions are typically required for optimal outcome.

7. Chemical Peels

Service: BioRePeel (35% TCA Bi-Phase Peel)

Description: BioRePeel is a next-generation bi-phase TCA peel containing 35% trichloroacetic acid (TCA), amino acids, vitamins, and GABA. It is an advanced modality performed only by trained staff with Management sign-off. It resurfaces the skin and stimulates collagen without the traditional downtime of a TCA peel.

Service-Specific Risks

  • Redness, warmth, and tingling during treatment

  • Mild sensitivity or skin tightness for 24-72 hours post-treatment

  • Possible mild flaking or shedding in the days following treatment as cellular turnover occurs

  • Temporary hyperpigmentation, particularly in higher fitzpatrick skin tones, if post-care instructions are not followed

  • In rare cases: prolonged redness, swelling, or allergic reaction

  • Contraindicated for: active skin infections, open lesions, or wounds; active cold sores or herpes simplex in the treatment area; isotretinoin use within six months; recent laser or ablative treatment within four weeks; pregnancy or nursing; known allergy to TCA or any listed ingredients; extremely compromised barrier or sensitized skin

Pre-treatment and post-treatment protocols are mandatory and will be communicated at the time of booking and confirmed at your appointment. Failure to follow pre- or post-treatment instructions may increase the risk of adverse outcome and may affect results.

Advanced Modality Notice

BioRePeel is an advanced treatment and is only performed after independent assessment and sign-off by the lead practitioner. We reserve the right to decline this service at the time of appointment if the practitioner determines that your skin is not in an appropriate condition to receive it safely.

8. LED Light Therapy

Included as a modality in multiple services

Description: LED light therapy uses specific wavelengths of light to support skin health. It is a non-invasive, non-thermal modality. It is incorporated into multiple JBS treatments and is not offered as a standalone service.

Service-Specific Risks

  • Generally well-tolerated with minimal risk for most clients

  • Contraindicated for: clients with photosensitivity conditions or disorders (including certain autoimmune conditions); clients taking photosensitizing medications (including certain antibiotics, retinoids, or NSAIDs); active skin cancer or precancerous lesions in the treatment area; epilepsy (for certain wavelengths)

  • Eye protection is provided and required during treatment

9. Waxing and Hair Removal Services

Description: Waxing involves the application of warm wax to the skin followed by removal of the wax and hair. Services may include facial waxing of eyebrows, upper lip, chin, and other areas.

Service-Specific Risks

  • Redness, irritation, or temporary inflammation in the treated area

  • Skin lifting, bruising, or abrasion if the skin is particularly thin or sensitized

  • Temporary ingrown hairs following removal

  • Contraindicated for: clients using isotretinoin (Accutane) within the past six months; clients using topical retinoids or exfoliants in the treatment area within 48 hours; sunburned or recently tanned skin; open wounds, active breakouts, or skin infections in the treatment area; clients with a known allergy to wax ingredients
